This policy will describe how Pacific Private Nursing collects, uses, and discloses personal information from website visitors, patients and their authorized representatives, and nurse applicants. See the Nurse Applicant Notice at Collection and general Notice at Collection for the categories collected through each specific form.
Counsel/owner to confirm before publication — this page must state actual practices, never assumptions:
- Identity of the verified operator and a privacy contact.
- Categories of personal information collected, described separately for visitors, patients/requesters, and nurse applicants.
- Actual sources, purposes, vendors, and required disclosures.
- Actual cookies, hosting logs, security tools, and analytics in use — never guessed.
- Actual retention periods and deletion practices.
- Security limitations and safeguards actually used.
- California privacy rights, if applicable, including request methods, verification, and authorized-agent procedure.
- A sale/share statement based on actual advertising-technology practices.
- Children's privacy statement.
- Policy-changes procedure and effective date.
Pacific will not claim HIPAA compliance on this website unless counsel confirms Pacific's legal status and a documented technical/compliance program supports the claim.
