This notice describes the personal information Pacific Private Nursing collects from California RN registry applicants through the public Registry Participation Application, and how that information is used, before you submit it.
Categories of personal information collected
- Identifiers: name, preferred name, email address, telephone number, city, postal code.
- Professional or employment-related information: California RN license number and expiration, years licensed, clinical experience description, specialties, service areas, general availability, BLS/CPR certification status, professional liability coverage status, referral source.
Do not submit identification, Social Security numbers, tax or banking records, medical information, patient records, or background-check documents through the public application. Pacific does not intend to collect these categories through this form.
Purpose of collection
- To evaluate interest in private-duty RN registry participation.
- To independently verify California RN licensure and publicly available disciplinary-status information.
- To contact the applicant about the application and, if applicable, an invitation to a separate secure credentialing process.

Retention
