This notice describes the personal information Pacific Private Nursing collects from website visitors and from patients or authorized representatives who submit the preliminary referral request, and how that information is used, before you submit it.
Categories of personal information collected
- Identifiers: requester name, email address, telephone number.
- Request details: relationship to the prospective patient, preferred contact method, service city, setting, approximate start date, general schedule, and an optional general-request description.
- [Counsel/owner to insert: whether hosting, security-log, cookie, and analytics practices apply once a production host is selected, and if so, which categories are actually collected. The production hosting provider and its logging configuration have not yet been selected or approved, so no such collection is stated as fact here.]
Do not submit diagnoses, medications, procedure details, photographs, insurance information, or medical records through the public preliminary request form. Pacific does not intend to collect these categories through this form.
Purpose of collection
- To respond to a preliminary inquiry about private-duty RN registry matching.
- To contact the requester about the inquiry and, if appropriate, next steps.
